Why am I getting a bill?
Your are getting a bill because your insurance company has responded to your claim. If you feel the information may be incorrect contact your insurance company before you contact the office.

Why do I get a statement from Evergreen Family Medicine and Evergreen Diagnostics?
Evergreen Family Medicine (EFM) and Evergreen Diagnostics (EDX) are two separate businesses even though they are owned by the same providers. Therefore, they have two different tax ID numbers.

When do I have to pay my bill?
Your payment is due upon receipt, if payment arrangements need to be established you must contact the business office to set up a payment plan. As a general rule all accounts should be cleared within 3 months.

Why is there a Boston address on my statement?
Evergreen Family Medicine has partnered with athenahealth for billing services. The charges are entered at Evergreen Family Medicine so your questions can still be directed to Evergreen Family Medicine.

How do I know if my doctor is contracted with my insurance?
Call your insurance, they are the ones that will be processing your claim and making payment.

Will my insurance cover a flu shot?
Only your insurance company can answer this question. There are hundreds of plans for each insurance company; many of which Evergreen is not aware of.

I would like to transfer care to Evergreen Family Medicine. How do I do this?
Call 677-7200 and we will give you the providers that currently have openings.

Who do I call when I need a prescription refilled?
Call your pharmacy for prescription refills unless the pharmacy requires a written prescription or you need a prescription to send to a pharmacy. If you are unsure if you need a written prescription, please call your pharmacy.

What if I need a same day appointment?
Call family practice first to check for openings and we’ll do the best that we can to get you in. If an opening isn’t available, we ask you to be seen in our urgent care.

Do I need to give advance notice when cancelling an appointment?
An appointment slot that is taken and not used means that someone else was unable to be seen. We would like to be notified just as soon as you know that you are unable to make the appointment so that we can schedule another patient

Urgent Care
Do I need an appointment to be seen?
Appointments are not made in the urgent care and patients are seen on a first come, first serve basis. Occasionally, a patient may be taken ahead of the others under certain circumstances.

What if I get a prescription in the urgent care and all of the pharmacies are closed?
The urgent care has a list of pharmacy hours in the event that a medication is needed.

Do patients need to fill out an New Patient Application?
No, we will supply you with the paperwork needed when you come in.
