Welcome to EVERGREEN FAMILY MEDICINE
Evergreen Family Medicine was born out of a desire to develop a system of care where patients of any age
had a primary care provider watching over their healthcare and guiding them through the healthcare system – in the
office, in the hospital, and with specialists. Evergreen’s founding doctors felt that this model represented the
best medical care for their patients.
Since 1996, Evergreen’s skilled and caring providers have delivered this model of care. Our Family Medicine
department cares for your entire family and provides a full range of care by appointment. Healthcare teams of Board
certified physicians, nurse practitioners, and physician’s assistants collaborate to bring our patients consistently
high quality care.
With our increasingly busy lifestyles, taking time to visit the doctor does not always fit into our schedules.
Evergreen Urgent Care is open 7 days a week to provide access to healthcare when you need it. Before work, after work,
or on the weekends, our physician led teams are ready for your bumps, sprains, and illnesses when they happen –
and we will return you to your busy life much faster than the emergency room.
Evergreen Diagnostics delivers laboratory, X–Ray and other diagnostic services on site with the same extended
hours as our Urgent Care. Nearly all of your routine and urgent diagnostic needs can now be met in one convenient
location by the same friendly staff.
